Insurance & Billing

All payments, including co-payments and unpaid balances, are due at the time of your visit, unless specific arrangements have been made in advance. We will ask you to verify your current address and insurance information at the time of each visit.

If your insurance plan requires a referral form from your primary care physician, then you must provide the completed form to us by the time of your appointment. Otherwise, you must assume responsibility for the full charges incurred at your visit or your visit may be rescheduled.  If you are insured by Medicare then you are required to pay the annual deductible and/or the 20% balance of Medicare’s allowed charges, either personally or by a secondary insurance.

We often need to send blood tests and biopsy specimens to outside laboratories, which will then bill you for their services. Please direct questions regarding those charges directly to the individual laboratory.  We also have a a pathology laboratory on site, and billing for these services is performed similarly according to the guidelines above.

Not all physicians participate in all plans. At GANJ, it is our mission to assist patients in aspects of their care including insurance and payment related matters. All insurance coverage is verified in advance and all claims to insurance carriers in which individual doctors participate will be submitted for assignment. Should patients be required to pay directly for services, we will as a courtesy, provide all the necessary paperwork to submit a claim for reimbursement.

Insurance coverage for lab work, pathology and anesthesiology services varies. Patients are responsible for any out-of-pocket expense as a result of co-pays, deductibles, co-insurance, etc.

We take most major insurances, Medicare and several smaller insurance plans.  Below is a list of the various insurance plans (with their websites) accepted at GANJ, but may not be inclusive.  It is subject to change, so please confirm with our office staff to ensure we accept your plan.   Questions regarding your account (with the exception of outside laboratories, hospital-related matters and the surgical center) should be made to our billing department (866) 362-6398.
